TAKING INTO ACCOUNT THE GROUNDS FOR EXCLUSION FROM ARTICLE 7(1) OF THE ACT ON SPECIAL MEASURES TO COUNTERACT SUPPORT FOR AGGRESSION AGAINST UKRAINE AND TO PROTECT NATIONAL SECURITY submitted pursuant to Article 125(1) 1 of the Public Procurement Law
For the purposes of cooperation with local government units and public administration, I hereby declare the following:
1. I declare that I am not subject to exclusion from the proceedings pursuant to Article 108(1)(1)-(6) of the Public Procurement Law.
2. I declare that I am not subject to exclusion from the proceedings pursuant to Article 109(1)(4) of the Public Procurement Law.
3. I declare that there are no grounds for my exclusion from the proceedings pursuant to Article 7(1) of the Act of 13 April 2022 on special measures to counteract support for aggression against Ukraine and to protect national security (Journal of Laws, item 835).
